Field trip permission slip: what to include, with a free template

A permission slip should say where, when and why the class is going, how they'll travel, the cost and what to bring, then collect the parent or guardian's consent, signature, date and an emergency phone number. Check your school's own form first; many schools require specific wording.

Tips that get slips back

  • Send slips two to three weeks ahead, with a clear return date.
  • Keep it to one page and use plain language; many families read it on a phone photo.
  • Two slips per page saves paper for simple local trips.
  • Keep a class list and tick slips off as they come in; send a reminder a few days before the deadline.
  • Have a plan for students who can't go, and say so kindly on the slip.

Rules and records

Schools and districts usually have their own permission form, insurance requirements and rules about chaperones, transport and medical information. Use the official form if there is one, or check that your slip covers what's required. Store completed slips as your school asks, and take copies with emergency numbers on the trip.

Other slips the same template works for

  • Photo and video consent for a class project.
  • Cooking or tasting activities (ask about allergies).
  • After-school clubs and sports.
  • Watching a film with an age rating above the class's age.

Day-of-trip checklist

  • Signed slips for every student going, plus copies of emergency contacts.
  • Medication and allergy plans, with the adult responsible for each.
  • A list of students in each chaperone's group.
